Declaration of Consent

a. The objective of the management system audit is to assess the implementation and effectiveness of the organization’s management system in accordance with the applicable management system standards and relevant regulatory requirements.
b. The purpose of the audit is to evaluate the organization’s compliance with the applicable management system standard(s) and the effectiveness of policies, procedures, and operational controls.
c. By signing this application form, the applicant confirms that all information provided is true and accurate to the best of their knowledge and agrees to cooperate fully during the audit process.
d. The applicant authorizes the auditing organization to review documents, inspect the facility premises, observe processes, and access relevant records, facilities, and personnel required for the audit.
e. The applicant acknowledges that participation in the audit process does not guarantee certification, approval, or endorsement, and that audit results will depend on the level of compliance demonstrated during the assessment.

Code of Ethics Agreement

The applicant acknowledges that IQMC operates under an Integrity Management Program and Code of Ethics in accordance with applicable professional and regulatory requirements. By signing this application, the applicant agrees to conduct all interactions related to the audit process in an ethical and transparent manner and confirms that no gifts, payments, favors, or inducements will be offered to auditors or representatives of IQMC. The applicant also agrees to immediately report to IQMC any solicitation, request, or offer related to bribery, corruption, or unethical conduct associated with the audit process. Failure to comply with these ethical requirements may result in suspension or termination of the audit process in accordance with the IQMC Ethics Policy.

Payment

Payment for the required services shall be made at least one day prior to the commencement of the audit services, unless otherwise agreed in writing.

Jurisdiction

This agreement shall be governed by and construed in accordance with the laws of India, and any disputes arising out of or in connection with this agreement shall be subject to the exclusive jurisdiction of the courts located in Meerut, Uttar Pradesh, India.

Acknowledgement

By signing this application form, the applicant confirms that all information provided is true, accurate, and complete to the best of their knowledge. The applicant further confirms that they have read, understood, and agreed to the Declaration of Consent, Code of Ethics Agreement, Payment Terms, Jurisdiction clause, and all applicable terms and conditions stated in this application form. The applicant agrees to cooperate fully with IQMC in conducting the management system audit and to comply with the applicable ethical requirements. I hereby confirm that I have read, understood, and provide my consent to the associated declaration.
